Proof of Citizenship for Passport Applications

 

Submit evidence of U.S. Citizenship with your application:

  • Current or expired U.S. Passport.
  • Original Naturalization or Citizenship Certificate.
  • Certified birth certificate issued by the State, City or County.

Obtain a Birth Certificate:

  • Only the long form of the birth certificate is accepted. Abstract Forms are not accepted.
  • For birth certificate information outside of Orange County, call the Santa Ana Public Library Reference Desk at (714) 647-5267 or visit VitalChek
  • Orange County Clerk-Recorder (For births that occurred in Orange County)

By Mail (Allow 5 - 10 business days for processing)

  • Print* and fill out all sections of the application:
  • Notarize the Sworn Statement on page 2 of the Application.
  • Include a check / money order made out to Orange County Clerk-Recorder for $21.
  • Send Application and check / money order to:

In Person (Same-day service)

  • Bring government-issued photo ID.
  • Check, cash or money order accepted.
