Proof of Citizenship for Passport Applications
Submit evidence of U.S. Citizenship with your application:
- Current or expired U.S. Passport.
- Original Naturalization or Citizenship Certificate.
- Certified birth certificate issued by the State, City or County.
Obtain a Birth Certificate:
- Only the long form of the birth certificate is accepted. Abstract Forms are not accepted.

- Orange County Clerk-Recorder (For births that occurred in Orange County)
By Mail (Allow 5 - 10 business days for processing)
- Print* and fill out all sections of the application:
- Notarize the Sworn Statement on page 2 of the Application.
- Include a check / money order made out to Orange County Clerk-Recorder for $21.
- Send Application and check / money order to:
